Online Profiles Service

You have received a login code to create a profile (or multiple profiles in case of multiple respons links). Please read the instruction below carefully first.

Instruction for filling in the questionnaire

1: Take a quiet moment in which you can fill in the questionnaire undisturbed.

2: Make sure you have the code(s) to hand.

3: Click on the link at the bottom of this page. If Dutch is not your native language, choose the language you feel most comfortable with and that you master best.

4: Enter the login code. Note: the dash in between is part of the code!

5: Enter the requested personal information.

6: Follow the instructions on the screen. If you use too much time, the connection will be broken and you will need to log in again.

The results will be discussed by appointment.
